Care Management
Consultation
- Every stage of life brings changes and decisions. But as we age we are often confronted with decisions we hoped we would never have to face. For most people making these kinds of decisions is extremely difficult and confusing. Having the support to sort through choices and find safe and appropriate options can be a relief. Individual counseling focusing on aging issues is available in a private office or special arrangements can be made for an alternate location.
- Similarly, children of aging parents often need a place to deal with issues that come along with changing relationship roles. It is hard to change from being the child to being the adult child- it is hard on everyone. Having a supportive ally or someone who is not a part of the family to help navigate the difficult decisions can be helpful in offering a different perspective.
- Sometimes the process involves the realization that there is not one right decision but rather a host of options to be assessed and sorted through. Supportive counseling and problem solving will focus on helping to make this period of transition easier.
Care Management
- Many adult children do not live near their parents or loved ones or are themselves managing busy careers that keep them from being able to manage the day to day issues that arise. Families often need ongoing support to help manage their parent's care. or check in on them regularly. Ongoing care management may involve attending medical appointments and having an overall perspective to manage and coordinate care options. It also includes looking at the overall picture of social and emotional support and how that can be maintained for the best quality of life possible. Care management needs are as different as every individual and so every effort is made to find a role that is most beneficial for all of us. Regular communication will be provided on an agreed upon basis.
Consultation
- Location or time constraints can be very stressful when it comes time to make family decisions around a change of living or care options for an aging parent. The first step in the process is to understand your and your loved one's needs. Research and consultation are available for an assessment of needs and referrals will be tailored to your specific situation. Home safety assessments and psycho social assessments are available within the confines of the current health parameters.
- Sometimes family members need an outside person with experience in aging issues to assist in sorting through the layers of logistics as well as emotions. Consultation around these issues is available in person or phone appointments.
- Finding the right situation for people in the midst of changing health and safety needs is challenging. Please call if you need assistance with finding the right kinds of services for your family situation.
